We accept referrals for adolescent girls presenting with the conditions below. Complex, co-occurring presentations are within our scope — the conditions we see most rarely travel alone.

How to refer a client
Call or email admissions
Reach Erin Unsworth, our Admissions Director, directly at erin@junhav.com or (435) 265-4245. You can also submit a referral inquiry through our admissions page. We pick up calls and respond to emails promptly.
Clinical intake call
A member of our clinical team will connect with you — and when appropriate, with the client and family — to gather clinical history, current presentation, risk level, insurance information, and any relevant records.
Records review
We'll request relevant records: prior assessments, therapy notes, psychiatric evaluations, and school records where appropriate. This helps us build an accurate clinical picture before admission.
Decision within 24–48 hours
We aim to give referring providers a clear clinical response within 24–48 hours of completing the intake call. If Juniper Haven is not the right fit, we will help identify appropriate alternatives.
What you can expect from us
Communication with outside providers
We maintain collaborative relationships with referring clinicians throughout the client's stay. If your client enters our care, you can expect regular updates (with appropriate releases) and a collaborative posture — not a handoff.
Continuity of care
Wherever clinically indicated, we coordinate with outpatient providers to ensure continuity of therapeutic relationships and treatment focus. We don't assume we know better than the providers who have history with your client.
Discharge planning from day one
We begin step-down planning early. Discharge summaries are sent to referring providers, outpatient therapists, and psychiatrists as appropriate. We don't discharge to a void.
Collaborative clinical approach
Our team is approachable and willing to consult. If you have questions about clinical formulation, treatment fit, or a specific case, our clinical director is available to talk through it with you.
Insurance we currently work with
Private pay is also accepted. Insurance verification is completed as part of the admissions process. If you have questions about specific benefit structures, contact admissions directly.
